## Growing Up in the Shadow of a Destroyed Rainforest

For most people, Papua New Guinea conjures images of dense, thriving rainforest. But for Theonila Roka Matbob, the landscape of her childhood told a very different story. Born and raised near the heart of Bougainville — the largest island in Papua New Guinea's Autonomous Region — she grew up surrounded not by towering trees and rich biodiversity, but by barren rock and sand.

"You have to travel miles into another region and territory just to find the trees, the forest," says Roka Matbob, now 35.

From her earliest years, the warnings were constant. Elders in her community cautioned her never to drink from the rivers, never to eat anything that had fallen to the ground, and never to venture near the water. The reasons, however, were rarely explained. It was only when Roka Matbob began asking questions of her own that the full picture came into focus — and what she discovered would shape the entire course of her life.

## The Mine That Scarred a Generation

At the center of Bougainville's environmental collapse sits the Panguna copper and gold mine, developed and operated by Bougainville Copper Ltd., a subsidiary of Rio Tinto — one of the world's most powerful mining corporations, headquartered across Australia and the United Kingdom.

Between 1972 and 1989, the Panguna mine extracted millions of tons of copper along with hundreds of tons of gold and silver. The economic rewards, however, came at a devastating cost. The mining operation brought in outside workers, displaced local communities, and channeled profits away from the island's Indigenous population. Tensions escalated into violence, eventually igniting a brutal civil war that lasted nearly a decade, claimed thousands of lives, and tore apart the social fabric of Bougainville.

Roka Matbob's own family was not spared. Just days before her third birthday, her father was seized by an armed group and subsequently killed. Her mother and siblings were left to survive as nomads, eventually seeking shelter in a government-controlled camp.

When the mine closed amid the unrest, there was no remediation plan, no environmental cleanup strategy, and no accountability for the lasting damage left behind.

"I was born into that broken environment," she says. "Growing up was a life on permanent survival mode."

## From Protest to Legal Action

Roka Matbob's activism began during her high school years, when she started organizing community protests. But she quickly recognized that local demonstrations alone would not be enough to hold a global mining giant accountable.

In 2019, she helped bring the Human Rights Law Centre to Bougainville to listen directly to affected community members. The outcome was a landmark report titled *After the Mine: Living with Rio Tinto's Deadly Legacy*, which documented in stark detail the human and environmental toll of the Panguna operation. When Rio Tinto acknowledged that they had never fully understood the on-the-ground impact of their mine, Roka Matbob saw it as a turning point.

She went on to become the lead complainant in a formal human rights complaint filed against Rio Tinto. The company responded within 24 hours of the complaint being lodged — a moment Roka Matbob describes as profoundly meaningful.

"It was a dream come true — the opportunity to represent the people's voice and speak directly to the stakeholder who changed our lives," she says. "I shed tears. Finally, my grandmother didn't get to do this, but I'm going to do that now."

In 2021, Rio Tinto agreed to fund an independent environmental assessment. By 2024, the company had signed a memorandum of understanding committing to work alongside impacted communities to address and remediate the damage caused by decades of mining.

## A Historic Recognition: The 2026 Goldman Environmental Prize

Roka Matbob's tireless campaign has earned her the 2026 Goldman Environmental Prize, one of the most prestigious honors in global environmental advocacy. The prize recognizes grassroots champions from each of the world's inhabited regions, and Roka Matbob was selected to represent the island nations.

"Theonila is leading a historic effort to obtain justice for decades of environmental and social devastation caused by the Panguna mine," said Ilan Kayatsky of the Goldman Environmental Prize. "Her efforts have brought together a coalition intent on improving the lives of Bougainvilleans, today and into the future."

Roka Matbob has indicated that she and her community will collectively decide how to use the prize money — a gesture consistent with the community-first philosophy that has defined her entire advocacy journey.

## Rooted in Culture, Driven by Motherhood

What sustains Roka Matbob through years of uphill struggle is a combination of deep cultural identity and maternal determination. She belongs to the Indigenous Nasioi people and the Basikang clan, a matrilineal society in which women are considered the traditional guardians of the land.

"We women are the land guardians and keepers," she says, citing a proverb from her community: *It takes a woman's tears to start a fight, and a woman's tears to broker peace.*

She also draws strength from her two young children, aged 8 and 4. "No mother would want to pass a broken, contaminated environment on to her child," she says. The knowledge that countless other children in her community lack advocates who can speak for them only deepens her resolve.

Roka Matbob has also carried her fight into the political arena, serving as one of a small number of women elected to Bougainville's House of Representatives. While she acknowledges that the political culture remains largely patriarchal, she views her presence there as both a challenge and an opportunity — a chance to ensure that the voices of women and Indigenous communities are no longer ignored in decisions that directly affect their land, their health, and their future.

## The Work Is Far From Over

Despite the hard-won progress, Roka Matbob is clear-eyed about what remains ahead. A memorandum of understanding is a beginning, not an ending. For communities still living in a contaminated environment, there is little room to pause and celebrate.

"When you are permanently in a broken environment, it does not give you space to stop and move on," she says. "The next question is: How soon can we fix it? How long is it going to take?"

Her story is a powerful testament to what sustained grassroots advocacy can achieve — even against some of the world's most powerful corporate interests. But for Roka Matbob, the Goldman Prize is not a finish line. It is simply more fuel for a fight she has no intention of walking away from.

For many people, getting a quick answer from an AI chatbot feels far easier than waiting weeks for a GP appointment. But as artificial intelligence becomes a go-to source of medical guidance, serious questions are emerging about just how trustworthy that advice really is.

### One Woman's Mixed Experience with ChatGPT

Abi, a Manchester resident who deals with health anxiety, has spent the past year consulting ChatGPT regularly about her health concerns. She finds the experience more reassuring than a standard internet search, which tends to surface worst-case scenarios immediately.

"It allows a kind of problem solving together," she explains. "A little bit like chatting with your doctor."

In one instance, the chatbot served her well. After describing symptoms that pointed to a urinary tract infection, ChatGPT recommended she visit a pharmacist, where she was assessed and prescribed antibiotics. For someone who struggles to judge when medical attention is necessary, Abi felt the AI helped her access care without unnecessarily burdening the NHS.

However, not every interaction went so smoothly. After a hiking fall left her with intense back pain radiating into her stomach, Abi turned to ChatGPT for guidance. The chatbot told her she may have punctured an organ and needed to go to A&E immediately. She spent three hours waiting in the emergency department, only for the pain to subside on its own. The AI, she concluded, had simply got it wrong.

### England's Chief Medical Officer Raises the Alarm

Abi's experience is far from unique, and it has caught the attention of England's most senior doctor. Professor Sir Chris Whitty, the Chief Medical Officer for England, has publicly voiced concern about the standard of health information being delivered by AI tools. Speaking to the Medical Journalists Association, he warned that people are already relying on these platforms even though the answers being generated are frequently "both confident and wrong."

As AI-powered responses increasingly appear at the top of internet search results, even those who aren't actively seeking chatbot advice may find themselves on the receiving end of it.

### What the Research Actually Shows

Scientists at the University of Oxford's Reasoning with Machines Laboratory have conducted one of the more revealing studies into this issue. A team of doctors constructed detailed, realistic medical scenarios spanning a wide range of severity — from minor issues manageable at home to emergencies requiring an ambulance.

When AI chatbots were given all the relevant information upfront, their diagnostic accuracy reached an impressive 95%. "They were amazing, actually, nearly perfect," said lead researcher Professor Adam Mahdi.

But the picture changed dramatically when 1,300 real participants were asked to have natural conversations with chatbots in order to receive a diagnosis. Accuracy plummeted to just 35%, meaning two out of every three people received incorrect diagnoses or inappropriate care recommendations.

The problem, Professor Mahdi explains, lies in how humans communicate. "When people talk, they share information gradually, they leave things out and they get distracted." Those gaps in communication are enough to send an AI down entirely the wrong path.

One particularly alarming example involved a scenario based on subarachnoid haemorrhage — a form of stroke caused by bleeding on the brain that constitutes a life-threatening emergency. Depending on how participants described their symptoms, ChatGPT offered wildly different responses. In some cases, it recommended nothing more urgent than bed rest — advice that could prove fatal.

Interestingly, Mahdi noted that participants who used a traditional internet search tended to land on the NHS website and were generally better informed as a result.

### The Hidden Dangers of Personalised-Feeling Advice

Dr Margaret McCartney, a GP based in Glasgow, highlights a psychological dimension that makes AI health advice particularly risky. Unlike a standard web search — where users can assess the credibility of a source based on the website it comes from — interacting with a chatbot creates the illusion of a personal consultation.

"It seems as though you're getting this supportive advice that's being made 'for you,'" she says, "and that probably changes the way we interpret what we're being told."

This false sense of personalisation may lead users to place far greater trust in AI responses than is warranted.

### AI Chatbots and Medical Misinformation

A separate study from The Lundquist Institute for Biomedical Innovation in California examined how prone AI chatbots are to spreading health misinformation. Researchers deliberately posed questions in ways that were likely to elicit inaccurate responses, testing the robustness of several leading platforms including Gemini, DeepSeek, Meta AI, ChatGPT and Grok.

The chatbots were assessed across five key health topics: cancer, vaccines, stem cells, nutrition, and athletic performance. The findings added further weight to growing concerns about the reliability of AI-generated health content.

### Should You Use AI for Health Questions?

AI chatbots are undeniably convenient, and in some situations — like Abi's UTI — they may point users in the right direction. But the evidence suggests their limitations are significant, particularly when it comes to complex or urgent medical situations.

Until the technology improves and appropriate safeguards are in place, health experts urge caution. If you are experiencing serious or worsening symptoms, consulting a qualified medical professional remains by far the safest course of action.

Today, scientific interest has been reignited. A landmark 2025 study published in the *Journal of the American Medical Association* found that a single dose of LSD could effectively reduce symptoms of anxiety and depression for several months.

Currently, both psilocybin and ibogaine remain classified as Schedule I substances under the U.S. Drug Enforcement Administration — a designation indicating no accepted medical use and a high potential for abuse. Trump's order is expected to fast-track the reclassification of both drugs. Hi there, how’re you? How’s it going? You alright? All good?

As any Briton knows, none of these questions is an inquiry into your emotional state, the material conditions of your life or your opinion on anything. Respond positively – “all good so far, touch wood” is nice – then move on to the purpose of the interaction: “I’m returning an Amazon package?”

I communicate for a living and am nearly always in front of a camera, microphone, keyboard or live audience. Great conversation is a supreme design to me, an exquisite dance of mutual pleasure practised by true proficients who make it seem effortless. And then there’s small talk , the Primark version for everyday wear. That’s all the chat around the edges, the bits of make-do and banter. Busy day so far? What’s the traffic like?

But make no mistake – this kind of conversation isn’t superfluous, it’s vital. In fact, the work day doesn’t flow without small talk. It’s team building, not forced conversation, and it should occur simultaneously with a necessary interaction, transaction or service, to ease the movement like linguistic synovial fluid.

Recent research backs me up . Across three countries (Singapore, the US and France) and 1,800 people, the Journal of Personality and Social Psychology reported that people find value in everyday conversations they’d anticipated as being boring. So small talk is a social good with a bad reputation. We dread it, but it benefits us.

Small talk is not an invitation to engage in deep conversation, theoretical debate or soul exchange, but a hedge against them. It is supposed to be rote GCSE-level role play about universal topics on which everyone has the same opinion, by which I mean the weather, not death. You are meant to recycle phrases that are not interesting, without introducing novel information.

Through small talk, this week I learned that a recent football match was like watching kids in a playground (ie both amateurish and boring), that April weather is so variable you’re never sure what coat to wear, and that now the clocks have gone forward the evenings are noticeably brighter. Through small talk, you can be polite to the dry cleaner for one minute, three times a year, for decades, saying the same thing over and over, and you can both earn a reputation as pillars of the community, however true or not that might be.

I mean, don’t force your inane babble on the unimpressed staff at the 24-hour shop, like some sad woman whose eventual departure is trailed by pitying looks and the shop workers saying: “Poor thing. She must be lonely.” Be brief, sincere and bland. Don’t ask weird questions. I was once asked by a serial-killery waiter what exactly I planned to do for the rest of the day. And I was once trapped on a night bus in a traffic jam in the rain, having some doleful chat with the woman next to me when she said, with intent: “It’s a bit grim, isn’t it? A bit like my year.” I did not take the bait. I was also asked by a very intense girl at a gallery cafe which painting I liked best in the exhibition, and why. I froze, then whispered: “I like all paintings equally.”

If you avoid these pitfalls, you’ll do fine. Small talk isn’t boring; it’s a necessary staple. It’s not meaningless blather; it’s essential reinforcement with deeply embedded cultural meaning. It’s a bulwark against total social breakdown. The one thing that human beings do that distinguishes us from other mammals is use language to communicate, express ourselves and understand others. Has evolution done a U-turn? Just take one look down the length of a train carriage, crammed with inert, slack-faced blobs staring at their phones, seemingly unaware of each other’s existence.

How have we got a society – it’s not just young people, I’ve seen it in all generations – in which we no longer know how to talk to each other with decency, self-awareness and consideration? Through small talk, we patch and mend a world, word by word, that hasn’t been completely ripped apart by rage, paranoid mistrust, fear and loathing or numb resignation. That is what I’m getting at when I ask if you’ve had a chance to get out of the office and enjoy the sunshine at all.

## A Welcome Step, But Far From Enough

The UK government's decision to relaunch its women's health strategy has been met with cautious optimism by many advocates. While the initiative signals a willingness to confront long-standing issues within the healthcare system, critics argue that the underlying causes of health inequality remain stubbornly intact — and that for women of color, the situation is particularly dire.

Vanessa Haye, a health advocate based in Chislehurst, Kent, acknowledges the strategy's intent but questions its depth. In her view, the system may appear to be listening, but structural barriers that have historically silenced marginalized women are still very much in place.

## The Staggering Reality of Women's Healthcare Gaps

The strategy does confront several critical issues that women have been raising for years. Gynaecology referral waiting lists have grown so long they would span over 191 miles if patients were made to queue in person. Medical gaslighting — the dismissal or minimization of women's symptoms by healthcare professionals — continues to result in delayed diagnoses and prolonged suffering.

Systemic bias within clinical settings remains a persistent problem, one that disproportionately affects women who are already vulnerable due to socioeconomic status, cultural background, or ethnicity.

## Women of Color Are Being Left Behind

Despite Health Secretary Wes Streeting's stated commitment to ensuring every woman's voice is heard, many women of color remain unconvinced. Reproductive health outcomes for ethnic minority women have seen little meaningful improvement, even as advocacy and outcry have grown louder over the years.

The experiences of these women paint a troubling picture. A common scenario involves visiting a GP with debilitating period pain, only to be told the symptoms are normal and handed a prescription for contraceptive pills. For many, it takes decades of repeated dismissal and medical gaslighting before a formal diagnosis — often of a chronic condition the woman had suspected all along — is finally made.

This is not an isolated story. It is the lived reality of thousands of women, including Haye herself, who spent years navigating a system that consistently failed to take her seriously.

## Ethnicity and Access Still Define Who Gets Heard

At the heart of this issue is a difficult truth: ethnicity, culture, and access to healthcare continue to determine who is believed, how quickly their concerns are acted upon, and what outcomes they ultimately receive. Without directly confronting these inequalities, any health strategy risks simply replicating the very disparities it claims to be solving.

Technology-driven solutions, often referred to as femtech, have been proposed as part of the answer. While innovation in this space holds genuine promise, it is not automatically equitable. When funding and development opportunities are concentrated among specific groups or founders, the resulting products may not serve the full diversity of women's needs.

Policy, too, is only as effective as the people shaping it and the direction in which it is steered.

## Misogynoir Must Be Named and Addressed

If the government is truly committed to tackling medical misogyny at its roots, advocates argue that it must go further — explicitly recognizing and addressing misogynoir, the specific intersection of racism and sexism that disproportionately harms Black and ethnic minority women within healthcare systems.

Building systems that are genuinely inclusive and reflective of the full spectrum of women's experiences is not merely an idealistic goal — it is a medical and moral necessity. Without this, the relaunched strategy risks becoming another well-intentioned initiative that falls short for those who need it most.

## Record Numbers of Young Men Are Staying at the Family Home

A growing financial squeeze is reshaping the way young adults in the UK live, with new figures revealing that more than one in three men aged 20 to 35 were living with their parents in 2025 — the highest proportion recorded since at least 2007. According to data published by the Office for National Statistics (ONS), 35% of young men now remain in the family home, a sharp rise from 26% back in 2000.

While young women are experiencing a similar trend, they do so at a lower rate, with 22% living with their parents in the same period. Notably, both figures have been climbing steadily in recent years.

## Soaring Housing Costs Are Driving the Trend

Researchers and economists point to sky-high rental prices and an increasingly unaffordable property market as the primary forces behind this shift. A study conducted by the Institute for Fiscal Studies identified housing costs as a major barrier preventing young adults from striking out on their own, with financial pressures making independent living feel out of reach for millions.

The ONS also found that the rising cost of living ranked as the single most pressing concern among UK adults — placing it above worries about the NHS and the broader economy.

### One Young Man's Story: Saving £50,000 by Staying Home

For some, the decision to stay put is a calculated financial strategy. Nathan, 24, lives with his father in a council house on the outskirts of Manchester. Working night shifts cleaning and maintaining trains, he has managed to accumulate an impressive savings pot of £50,000 — a feat he attributes entirely to not paying market-rate rent.

"If you are lucky enough to live with a parent or grandparent who doesn't charge you much, you can build up money even on minimum wage," Nathan explains.

His approach to saving is disciplined and deliberate. He batch-cooks hearty meals like chillies and stews to bring to work, avoids impulse buying, and caps his spending on a night out at around £20.

"I'm not interested in a flashy car, blowing money on alcohol, or buying £500 trainers," he says. "My dad bought a house at 21, but that was just how things were back then."

### Returning Home After University

For others, moving back in with family is less of a choice and more of a necessity. Harry Turnbull, 22, returned to his mother's home in Surrey last summer to complete his degree after spending three years living independently with friends on the south coast.

Despite missing the freedom of student life, Harry says the financial reality left him with little alternative.

"Everything is just so expensive, and prices have gone up massively," he says. "Young people should have more of an option to live independently, but right now that simply isn't possible for a lot of us."

## How UK Households Are Changing

The ONS data paints a broader picture of shifting household dynamics across Britain. Roughly three in ten UK households now consist of a single person living alone, with nearly half of those — approximately four million people — aged 65 or over, a slight increase compared to 2015.

At the same time, the proportion of households made up of married couples, with or without children, has declined as alternative family structures — including unmarried partnerships, civil unions, and single-parent households — become increasingly common.

## The Wider Cost of Living Crisis

The financial pressure facing younger generations sits within a much larger national context. Separate ONS surveys conducted in early 2026 found that more than two thirds of adults reported that the cost of living had increased in March compared to the previous month, with rising food and fuel prices cited most frequently as the cause.

Between January and March of the same year, nearly one in four adults said they would be unable to cover an unexpected but essential expense of £850. A further 3% reported having run out of food in the preceding two weeks and lacking the funds to buy more — a stark illustration of the financial vulnerability facing a significant portion of the population.

For young people navigating this landscape, the family home has become less of a stepping stone and more of a lifeline.

A rear admiral in the US Coast Guard, Schwartz previously served as deputy surgeon general during Trump’s first administration. Over more than two decades, she has held positions in the US navy, the Public Health Service Commissioned Corps and the US Coast Guard. Her tenure as deputy surgeon general lasted from 2019 to 2021.

Schwartz is a physician with a background that includes a bachelor’s degree in biomedical engineering and a medical degree from Brown University. She also has a master’s degree in public health from the Uniformed Services University of the Health Sciences, as well as a law degree from the University of Maryland.

Jay Bhattacharya had been serving as acting CDC director, but that designation expired last month under federal law. The statute, known as the Vacancies Act, restricts acting officials from serving longer than 210 days in roles that require Senate confirmation.

Since Trump returned to office in January, Robert F Kennedy Jr, the US health secretary, and his team have taken public and private steps aimed at slowing vaccine research and guidance.

The CDC’s last Senate‑confirmed director, Susan Monarez, took over in July but was fired less than a month later after clashing with Kennedy over his anti-vaccine agenda.

Protect Our Care, a health policy group founded by a former Obama administration health official the day after Trump won the 2016 presidential election, has called on Schwartz to promise that she will stand up for the safety and effectiveness of vaccination.

Kayla Hancock, the director of Protect Our Care’s public health project and a former head of opposition research for the Democratic National Committee, said in a statement: “For the sake of our public health, the next CDC director must be free and independent to encourage as many Americans as possible to protect themselves from preventable diseases without first getting permission from the anti-vaxxer-in-chief.”

During a staff meeting in March, Bhattacharya pointed to several difficulties the agency had encountered over the past year, including repeated layoffs, declining staff morale and an incident in which a gunman fired outside the CDC’s Atlanta campus, resulting in the death of a police officer.

"That very quickly turned into self-destruct mode," he admitted. "Deliberately not injecting insulin is extremely dangerous, but I just didn't care."

Duke had previously been diagnosed with obsessive compulsive disorder (OCD), and he describes the arrival of his diabetes diagnosis as something that "collided dramatically" with his existing condition. For Duke, OCD manifests as intrusive thoughts centred around numbers and repetitive actions, compelling him to perform mental compulsions in an attempt to prevent feared outcomes.

"It's like a super-glued grip on your brain," he described. "If you don't perform the compulsion, you're convinced something terrible will happen — and that it will be your fault."
