The Trump administration issued an executive order earlier this month to accelerate access to psychedelic medication for people with “serious mental illnesses”, but experts say the order is more likely to make a difference symbolically than legally.

“Policymakers and the medical field have long struggled to address the burden of suicide and serious mental illness rates in America,” the order reads, noting that some people do not respond to available treatments.

The order then lists provisions to accelerate access to psychedelic medications, including inter-agency collaboration with the Department of Veterans Affairs to share data and increase clinical trial participation, and timely rescheduling in the case that an illegal psychedelic receives FDA approval.

Mason Marks, a law professor at Florida State University and senior fellow for psychedelic projects at Harvard Law’s Petrie-Flom Center, said that while the concrete provisions of the order will probably have only “some minimal impact”, he added that “the overall order itself could have a pretty significant impact, just in terms of its symbolic value”. Marks noted that the order came with a lot of fanfare, including a press conference with Joe Rogan, a podcaster, and Marcus Luttrell, a former Navy Seal.

“It’s quite unusual for a president to be promoting psychedelic research in such a public way,” Marks said. During the conference, Rogan said that a text conversation between himself and the president inspired the executive order.

Cat Packer, director of drug markets and legal regulation at the Drug Policy Alliance, said the order is also a signal of “who is influencing the administration”, noting that Republican support for psychedelics is expanding and includes elected officials, veteran advocates like Luttrell and “high-profile public figures with proximity to the president”, such as Rogan.

“It’s a welcome sign that the federal government is taking psychedelic research and access seriously,” Packer said, before cautioning that the same framing that seems to have made psychedelic medications more popular with some Republicans could also limit who benefits from legal changes.

Packer continued: “The administration is clearly leaning into a medicalized, veteran-centered framework, rather than a broader rights-based or anti-criminalization approach. Without sustained investments in healthcare access , community-based treatment and housing – resources the Trump administration has continued to divest from – we cannot truly address America’s mental health crisis.”

Trump issued a similar executive order in December directing that cannabis be reclassified from a schedule I to a schedule III substance, which would remove restrictions on research as well as potentially easing some of the tax burdens that come with selling it. The Biden administration had initiated the rescheduling process in 2022 .

“There’s already been some evidence of institutional resistance when it comes to cannabis rescheduling. President Trump himself has suggested that parts of his administration have stalled the process – likely reflecting the reality that the [US Drug Enforcement Administration] has historically resisted efforts to reschedule marijuana,” Packer said.

Marks echoed that sentiment: “The reason I think the psychedelic order is different is that we haven’t seen anything like this from the White House.”

There are a few other reasons that the psychedelics order is more promising. While the cannabis order had one essential provision – rescheduling – this one includes more straightforward directives, Packer said, like funding state-level research. Plus, Marks said, “there’s no question the FDA is very open to this. We are on the cusp of seeing FDA approval potentially for a handful of psychedelics. I would put that somewhere in the one-to-three-year range.” Drugs that are far along in the process include psilocybin for treatment-resistant depression.

That these drugs are already close to approval render some elements of the order somewhat meaningless. For example, while the order directs the Drug Enforcement Administration to reschedule any drug that successfully completes a phase 3 trial, Marks pointed out that the agency is already obligated to do that within 90 days. “You can’t get much faster than that,” Marks said.

The order also indicates that the FDA’s safety review process should be accelerated. Marks is concerned this provision will require reviewers to employ AI to help accelerate the process, which could lead to missed adverse events. Psychedelic clinical trials have already come under scrutiny for failing to report adverse events .

“The problem is that if they miss something, and then some adverse event crops up, after the drug is out in the market, it might have to be recalled, and that’s a horrible scenario, it really sets back everything,” Marks said.

Packer agreed, saying: “Faster review and expanded research can be positive, but only if they are paired with rigorous evidence, strong patient protections and a broader public-health approach. It’s also important to note that while these substances hold promise, they are not a panacea.”

About one in 10 operations in England are cancelled with less than 24 hours’ notice or postponed, according to research..

A study of elective surgery at 91 English NHS trusts found that 10% of operations were cancelled the day before the planned surgery date; while 9% were postponed when patients had their pre-op appointment.

If the study’s findings were replicated nationally, that would equate to approximately 300,000 cancellations or postponements. Yet nearly 40% of cancellations could be avoided, the authors concluded.

Researchers for the National Institute for Health and Care Research Central London patient safety research collaboration, NHS England, University College London and the Royal College of Anaesthetists examined planned surgery data over seven days in November 2024. They found that the most common causes of cancellations were for medical reasons, patients not attending, operating lists overrunning and emergency admissions. But in 37.3% of cases, had these issues been identified as little as three to five days earlier, the operation could either have gone ahead, or another patient could have been offered the surgery slot, the study calculated.

The study, published in the British Journal of Anaesthesia, also found that nearly two-thirds of operations postponed at the pre-op appointment were because patients needed further tests or specialist clinical review.

The authors concluded that clinical pathways need overhauling, with more early screening, nimbler surgery scheduling and better communication.

The lead author, Dr James Bedford of University College London, said: “We need to ensure we identify health problems, which put patients at risk of post-operative complications, as early as possible, so that these can be improved while they are waiting for their operation.”

Early screening also helps to identify low-risk patients who could be offered surgery at short notice, when slots become free at the last minute, he added.

Writing in a linked editorial, Prof Scarlett McNally, a consultant orthopaedic surgeon, said the findings “underscore the scale of systemic inefficiencies, unacceptable waste of public money and emotional toll experienced by patients”.

She said: “Without a different approach that focuses on supporting and preparing these patients, waiting lists will remain unacceptably high, as procedures are too frequently postponed or cancelled.”

Prof Frank Smith, the vice-president of the Royal College of Surgeons of England, said the study underlined just how damaging cancelling or postponing operations could be for patients and the NHS.

He said: “High-quality care before and after surgery is just as important as the operation itself. The findings underline the need for earlier, better coordinated care so patients are in the best possible condition when they reach the operating theatre.

“Supporting patients to ‘wait well’ reduces last-minute cancellations, improves recovery and helps the NHS make better use of precious surgical capacity.”

An NHS spokesperson said: “We are seeing great examples of NHS teams across the country offering more personalised support to ensure patients are fit for surgery, and we need to continue and extend this progress to improve preoperative care for patients, eliminate avoidable postponements and keep bringing waiting lists down.”

AASA conducted a member survey that found the majority of school districts reported they would struggle to absorb the costs associated with full compliance by the original deadline.

"We are outraged," said Corbb O'Connor, president of the National Federation of the Blind of Minnesota. "Yet again, the blind have been told to wait to live on terms of equality."

Jennifer Mathis, who served at the Justice Department when the original rule was drafted and helped develop it, offered a particularly pointed critique of the reversal. "The whole point of this particular rule was to create certainty and clarity for everyone," Mathis said. "To delay the standards now, after 16 years and an incredibly thorough rulemaking process, is just mindless and cruel."

With the peak hay fever season typically arriving in May and June, allergy sufferers could face even greater cost pressures in the coming weeks.

Olivier Picard, Chair of the NPA, highlighted the dramatic shift in wholesale pricing at his Berkshire pharmacy. The cost of a pack of 100 500mg paracetamol tablets surged from just 41p to £1.99 by the end of March before easing slightly to £1.09. At the retail level, one pharmacy that charged £1.19 for a 32-tablet pack before the conflict broke out now charges £1.50 for the same product.

For cetirizine, Picard's wholesale purchase price has nearly doubled since January — rising from 19p per 30-tablet pack to 37p — while some distributors are charging as much as £3 per pack.

## Why Is the Iran War Driving Up Medicine Prices?

The conflict in Iran has triggered a cascade of economic consequences that are rippling through the pharmaceutical supply chain. The sharp rise in petrol and diesel prices since the war began has significantly increased both manufacturing and transportation costs for medicine suppliers, who are now passing these elevated expenses on to pharmacies. Community chemists are reportedly paying 40–50% more when ordering stock from wholesalers.

Air freight costs have also doubled since the conflict began — a particularly concerning development given that one in every five NHS medicines is transported by air. Furthermore, the war has severely disrupted the supply of petroleum derivatives from the Gulf region. These chemical compounds are essential raw ingredients in the production of many common medications, including paracetamol, aspirin, and co-codamol.

Manufacturers of generic, off-patent drugs — who typically operate on thin profit margins — have begun raising their prices to offset these rising input costs, creating a knock-on effect that inflates both NHS spending and retail pharmacy prices.

## NHS Reimbursement Rates Failing to Keep Pace

Community pharmacies derive roughly 90% of their revenue from dispensing NHS prescriptions, for which they are reimbursed at fixed government-set rates. While the government can grant price concessions when the cost of a medicine rises substantially, the current system is struggling to keep up with the pace of recent increases.

In March, a record 230 medicines were added to the price concessions list — more than double the 90 items listed in the same month the previous year. The list now includes blood pressure medications, anxiety drugs, antidepressants, and painkillers such as codeine and co-codamol.

However, paracetamol — one of the most prescribed medicines in England, with 1.3 million packs dispensed every month — was not included on the concessions list. The government currently reimburses pharmacies just 49p for dispensing a prescribed 32-pack, a figure that falls well short of current wholesale prices. Cetirizine was similarly absent from the list.

"The cost of medicine is soaring and pharmacies end up dispensing at a loss," Picard warned, noting that 1,400 pharmacies have closed since 2020 and the rate of closures continues at one to two per week.

## Wider Implications for the NHS and Global Health Systems

The rising cost of generic medicines is not only squeezing community pharmacies — it is also inflating the NHS medicines bill at a time when healthcare budgets are already under considerable strain. Health systems in other countries are facing similar pressures.

Mark Samuels, Chief Executive of Medicines UK — which represents manufacturers supplying 85% of NHS prescriptions — acknowledged that widespread shortages have been avoided so far, largely because of existing stock held in UK warehouses. However, he cautioned that this buffer will not last indefinitely.

"Transportation costs have risen by several hundred per cent, and some chemicals needed for manufacturing are in very short supply," Samuels said. "If the conflict continues, we will inevitably see rising prices or shortages of essential medicines. This could be as soon as the next few weeks."

While supermarkets and online pharmacies have historically offered cheaper generic drug prices, the same supply chain pressures are expected to push their prices upward as well. More expensive branded medication versions could see even sharper increases.

## What Should Patients Do?

Despite the concerning outlook, the NPA is urging patients not to panic-buy or stockpile medications. Picard stressed that bulk purchasing behaviour would only deepen existing shortages and drive prices higher for everyone. Patients are encouraged to purchase what they need and consult their pharmacist or GP if they have concerns about access to specific medications.
