For Layla, a 30-year-old teaching assistant, the day doesn't begin without a carefully engineered sequence of phone alerts. Four alarms to wake up. One to get dressed. Another to pack her bag. The list goes on.
"There's a million different ones. Different times of the day," she explains, scrolling through the extensive roster of reminders on her phone.
To outsiders, it might seem excessive. But for Layla, these structured checkpoints are essential tools for managing her ADHD — helping her brain stay on track and maintain a predictable routine.
That carefully built sense of order, however, tends to crumble the moment her period approaches.
"It's like you're clinging on, for dear life, onto the ball that is continuing to roll, and sort of cursing yourself and the world the entire time," she says.
What Is ADHD — and Why Does It Affect Women Differently?
Attention Deficit Hyperactivity Disorder, commonly known as ADHD, is a neurological condition in which the brain processes information differently from the majority of people. According to the NHS, it can involve difficulties with concentration, emotional regulation, and impulse control, among other challenges.
Approximately 2.5 million people in the UK are believed to live with ADHD, with hundreds of thousands still waiting for an official NHS diagnosis. For decades, ADHD was considered primarily a childhood condition — and one that mainly affected boys. It wasn't until the late 1990s that the medical community began to seriously acknowledge its presence in adults, and particularly in women.
"We're seeing increasing numbers of women," says Dr. Catherine Durkin of the Royal College of Psychiatrists. "Thanks to increasing awareness of how ADHD symptoms can present in women, more are recognising where they have struggled throughout their lives and are now asking for help."
This growing awareness has triggered an unprecedented rise in demand for ADHD assessments. NHS data from December of last year revealed a 23% increase in stimulant and ADHD-related prescriptions compared to the year prior. Yet a government taskforce report published in November found that the condition remains significantly under-diagnosed and under-treated — with some NHS services in England forced to close their waiting lists entirely due to overwhelming demand.
Groundbreaking Research Explores the Period–ADHD Connection
While online communities and social media forums have long buzzed with accounts of menstrual cycles making ADHD symptoms worse, the scientific community is only now beginning to formally investigate this relationship.
A pioneering study led by researchers at King's College London and Queen Mary University of London is addressing this gap head-on. The study involves 50 women who have been diagnosed with ADHD and are currently on medication. Participants are tracking both their menstrual cycles and the severity of their ADHD symptoms over time, providing researchers with a detailed picture of how hormonal shifts influence daily functioning.
Participants have maintained electronic diaries documenting hormonal changes alongside their experiences of anxiety, depression, impulsive behaviour, and overall cognitive performance. Researchers are analyzing this data to identify patterns — both in symptom intensity and in how effectively ADHD medications perform at different phases of the cycle.
When Medication Stops Working
Héloïse, a 19-year-old juggling three simultaneous university degrees, describes her ADHD medication — Ritalin — as an absolute lifeline. It typically provides her with a focused three-hour window for studying. Her routine is precise: take the pill, head to the library, and wait for clarity to arrive.
During her period, however, that clarity never comes.
"It feels like losing a walking stick or something you use to support yourself. All of a sudden it's broken," she says.
It was only after joining the King's College study and methodically mapping her symptoms that Héloïse recognised the unmistakable pattern connecting her hormonal cycle to her ADHD experience. Other women in the research echoed her sentiment, with several describing themselves as feeling "disabled" by their symptoms at specific points in the month.
The Science Behind the Hormonal Disruption
Psychiatrist Dr. Sally Cubbin, a neurodiversity specialist involved in the King's College study, believes the relationship between hormones and ADHD is deeply intertwined — and largely overlooked by mainstream medicine.
She explains that in the days leading up to menstruation, oestrogen levels drop significantly. This hormonal dip directly affects dopamine — the brain's so-called "feel-good" chemical — which plays a central role in focus, motivation, and impulse control. When oestrogen falls, dopamine function is disrupted, and ADHD symptoms can intensify sharply.
The consequences extend beyond difficulty concentrating. Dr. Cubbin notes that women are also more prone to poor decision-making and risk-taking behaviour during these hormonal low points — including binge eating, impulsive spending, and even neglecting contraception.
For Layla, this resonates deeply. She struggles with intense food cravings and overeating during her period. "I love pickles. I know that if I eat too many pickles I'm gonna feel ill," she admits. "It's not because I'm hungry. It's this impulse to do it — and once it's in your head it's really difficult to shut it down or turn it off."
A New Frontier in Women's Health Research
Dr. Jessica Agnew-Blais, the lead academic on the King's College project, emphasises that ADHD in adult women represents one of the most underexplored territories in current medical research.
Participants and researchers alike are also raising questions about the impact of larger hormonal transitions — specifically menopause and perimenopause — on ADHD symptoms. Dr. Agnew-Blais acknowledges that while her study is not yet complete, it could serve as a valuable "starting point" for exploring those connections.
The ultimate goal, she stresses, is not to alter who someone is. "It can help just fit ADHD into your life a little bit better," she says.
Finding Validation in the Data
For Layla, participating in the study has been more than just a contribution to science — it has been personally transformative. She admits she has spent years criticising herself for not "coming up to scratch," internalising her struggles as personal failings rather than symptoms of a condition influenced by biology.
Being part of the research, she says, has felt "validating and freeing" — a reminder that the challenges she faces each month are real, measurable, and deserving of attention.
As awareness continues to grow and research catches up with lived experience, millions of women may finally find the answers — and the support — they have long been denied.
